Minor Forms:   Parent/Guardian must accompany ALL minor clients.

1. Valid Texas Driving Permit held a minimum of 6 months and 16 years of age+
2. 2 Hour DPS Impact Certificate
3. Course completion from driver education school DRIVER LICENSE ONLY COPY
4. 30 Hour log signed by parent/guardian

Adult Forms: (under 25 years of age)

1. Valid Texas Driving Permit
2. ADE 6 Hour Adult Driver Education Form from driving school
3. 1 Hour Texas DPS Impact Certificate
